How QuoteForms Work πŸš€

All QuoteForms follow this structure:

  1. Applicant Information (required on all forms)

  2. Line of Business Selection

  3. Line of Business–Specific Questions

  4. Conditional questions that appear based on selections

Fields dynamically show or hide depending on client answers.

Applicant Section (Common to All Forms)

These fields appear on every QuoteForm:

  1. First Name

  2. Last Name

  3. Phone Number

  4. Email

  5. Best Method of Contact

  6. Address (Street, City, State, Zip, Country)

Home Insurance QuoteForm

The Home form uses a step-by-step layout to improve completion rates.

Step 1 – Property Address

  1. Is this a New Purchase?

  2. Use Primary Address? (if not a new purchase)

  3. When do you close on your new home? (if new purchase)

  4. Property Address

Conditional Logic

  • If New Purchase = Yes, closing date is required.

  • If Use Primary Address = No, property address must be entered manually.

Step 2 – Property Details

  1. What is the usage of this home?

  2. Do you ever rent this home to others?

  3. What is the shortest rental period? (if rented)

  4. Who manages the rental property? (if rented)

  5. Is the home rented on AirBNB/VRBO? (if rented)

  6. Year Built

  7. Square Footage

  8. Stories

  9. Bedrooms

  10. Full Bathrooms

  11. Half Bathrooms

  12. Roof Material

  13. Roof Shape

  14. Type of Primary Heating

  15. Number of Heating Units

Step 3 – Updates

  1. Have any of the following been updated?

  • Roof

  • Electrical

  • Plumbing

  • Heating & Cooling

If selected, follow-up questions appear:

  1. Roof Update Year

  2. Roof Update Type

Step 4 – Discounts

Step 6 – Swimming Pool

Step 7 – Trampoline

Auto Insurance QuoteForm

The Auto form collects information in three major sections.

Step 1 – Garaging Address

  1. Use Primary Address?

  2. Garaging Address (if not using primary)

Step 2 – Drivers (Repeats Per Driver)

For each driver:

  1. First Name

  2. Middle Name

  3. Last Name

  4. Date of Birth

  5. Gender

  6. Relationship to Primary

  7. Marital Status

  8. Education

  9. Occupation

  10. Driver’s License Number

  11. Driver’s License State

Step 3 – Vehicles (Repeats Per Vehicle)

For each vehicle:

  1. VIN (optional if unknown)

  2. Year

  3. Make

  4. Model

  5. When did you purchase this vehicle?

  6. Ownership Status (Owned / Financed / Leased)

  7. Does the vehicle have existing damage?

  8. How do you use this vehicle?

  9. Estimated Annual Miles

  10. One-Way Miles (if Work or School use)

  11. Used for ridesharing?

  12. Primary Driver for this vehicle
