Custom Fields & Question Order
The RiskProfile form is customizable — you can tailor the questions your clients see, add new ones specific to your practice, and arrange them in any order that suits your workflow. This article walks you through everything you need to know.

Overview
The form editor lets you:
Edit existing default questions visibility and requirements
Add custom questions with a variety of response types
Drag and drop questions to reorder them
All changes are saved to your account and apply to new form submissions going forward. Previously completed submissions are not affected.
Accessing the Form Editor
Log in to your account and navigate to Settings using the dropdown in the top right.
Select Settings from the menu.
Click Form Settings to open the editor.
You'll see a list of all current questions in the order they appear to clients on each Line of Business and section in the RiskProfile.
Adding a New Question
To add a question to the form:
Click + Add Question at the bottom of the question list (or between any two existing questions using the + Add Field button that appears when you hover between them).
A pop up modal will appear - this is where you choose your options.
Field Type — what type of question will appear. Each will have different formatting requirements. The options are Text, Number, Email, Phone, Dropdown, Money, Date, Yes/No, TextArea, Checkbox, and Percent.

Field Configurations - This is how the question will appear to users and how you set conditional logic (ex: only appear if a previous question is answered a specific way)

Visibility and Behavior — Determine if you want the field to appear and how users, if required, interact with it. Set state restrictions and placeholder text here.

Hit “Save Field” to add your new question!
For Dropdown and Checkbox questions, add your answer options using the + Add Option button.

Tip: If you want a new question to appear in a specific position, add it first and then reorder it — or click the + hover button at the position where you want it inserted.
Reordering Questions
You can drag and drop any question to a new position:
Hover over a question row — a drag handle (⠿) will appear on the left side.
Click and hold the handle, then drag the question up or down to the desired position.
Release to drop it in place. Make sure to hit “Save” when you’re finished in each section so your changes are applied.
Editing an Existing Question
Click the question label or select Edit to dive more deeply into the options
Make your changes in the modal.
Click Save to apply.
Note that editing a question's text or response type does not affect how previously submitted answers are stored.
