What (Custom) QuoteForms does

  • Turns individual fields on or off. Keep the questions relevant to what you're quoting, so a client's first impression is a form that already understands their situation.

  • Marks fields as required so clients cannot submit without answering them.

  • Gives each form its own web address. Every Custom QuoteForm gets a unique link, so you can hand a client the exact right starting point, by text, email, or embedded on your site.

  • Feeds submissions into a RiskProfile. Completed forms populate a RiskProfile in one-click.

A note on follow-up questions: QuoteForms supports conditional questions, so follow-up questions can appear based on earlier answers. To review how they are structured, see Manage conditional questions in the QuoteForm builder.

Here's how it works

  • You configure the form. Choose which fields appear, which are required, and how follow-up questions branch.

  • The client fills it out. They see only the fields you turned on.

  • It becomes a RiskProfile. Their answers populate a RiskProfile for your team to open.

Warning: The Email field drives client matching and RiskProfile creation. If you hide the Email field, no RiskProfile is created when the form is submitted. The form submits without error, there is no in-app warning. Keep the Email field visible to ensure every submission produces a RiskProfile.
