QuoteForms Setup Guide: Create, Customize & Share Your QuoteForms

Create, customize, and share QuoteForms, including how prospects submit their information and how you follow up.

QuoteForms collect structured applicant, property, and vehicle information directly from your prospects. Your team does not re-key any of it. When a prospect completes a form, you get a new Client and RiskProfile pre-filled with everything they entered. You can create unlimited QuoteForms and tailor each one to a workflow, line of business, team member, or referral partner.

Create a QuoteForm

  1. Go to QuoteForms in your dashboard, or visit app.riskadvisor.insure/quoteforms.

  2. Click Create a New QuoteForm.

  3. Enter the Name. This is internal only, so prospects never see it.

  4. Optionally add a Title, Subtitle, and Description to welcome prospects on the form itself.

  5. Set Assigned To. This user is notified when the form is completed.

  6. Choose the PolicyLink to offer for "Import Your Insurance," or provide a custom URL if you use Canopy Connect, InsurGrid, or another provider.

  7. Confirm the Lines of Business. Home and Auto are included by default. Add others as needed.

  8. Set the Confirmation Page: default message, a custom message, or a redirect to your own URL.

  9. Review Submission Protection (Bot Protection, reCAPTCHA) before publishing.

  10. Set Lead Source (from your AgencyZoom list, or free text if you don't use AgencyZoom).

  11. Click Create QuoteForm, then copy the shareable link or embed code.

πŸ’‘ Recommendation: Turn on Bot Protection and reCAPTCHA together with for any QuoteForm you plan to embed publicly.

Customize your QuoteForm

  • Description formatting. The description field supports basic HTML (<strong>, <i>, <u>) so you can bold, italicize, or underline part of your welcome message.

  • Property Data prefill. Enabling Property Data lets the Home QuoteForm pre-fill fields like pool, year built, square footage, and stories automatically.

  • Confirmation Page. You set this per QuoteForm, not globally, so different links can send prospects to different next steps.

  • Starting Page. To bypass the Entry Page and go straight to the Line of Business selection replace /respond with /start. This works for both standalone links and embedded QuoteForms!

Share your QuoteForm

There are two ways to share.

Option 1: Share a link

  1. Go to QuoteForms.

  2. Locate the form.

  3. Click the Link icon to copy the URL.

  4. Send it by email, text, or paste it into a CRM template.

Option 2: Embed on your website

  1. Go to QuoteForms.

  2. Click the Embed Code icon (</>).

  3. Paste the embed code into your site's HTML editor.

  4. Save or publish.

Note: If embedding on a public page, turn on Bot Protection and reCAPTCHA.

Skip the Starting Page: replace /respond with /start in either the link or the embed code to send prospects straight into the form. This is useful for targeted campaigns where you already know the line of business.

Best practices

  1. Your website. Use a dedicated link for your "Get a Quote" page for clean attribution.

  2. Per team member. Use a personal link per producer or CSR, with their profile picture uploaded for a personal handoff.

  3. Per referral partner. Use a link per partner so you can track and thank them.

Never miss a lead with Form Abandonment

Turn on Form Abandonment Recovery (QuoteForms β†’ gear icon β†’ toggle) so that if a prospect starts but doesn't finish, you still get a Client profile, a draft RiskProfile with whatever they entered, and an email notification with a direct link to follow up.
