Set up a Custom QuoteForm that asks clients only what you need. You choose the questions, the settings, and the Thank You page.
Create a form
Open Custom QuoteForms and click + New QuoteForm.
Enter the form's name and select the lines of business to include.
Click Create QuoteForm. The form builder opens.
Note: Custom QuoteForms is in open beta. If you do not see it, see Join the Custom QuoteForms beta.
Choose which questions clients see
The builder lists the form's sections on the left, such as Contact, Home, Auto, Umbrella, and Final Details. Click a section to open it, or click Next section to move through them in order.
For Home, Auto, and Umbrella, every question in the agent-facing RiskProfile is available on your form.

Turn questions on or off
Each question has two toggles:
Visible shows or hides the question for clients.
Required makes clients answer before they can move to the next page.
Note: A question must be visible before it can be required. If you hide a question, its Required toggle turns off.
To turn every question on a page on or off at once, use the All questions toggle at the top of the page.
Note: Email is always required. RiskAdvisor uses it to match the client and create their RiskProfile.
Tip: The defaults are a starting point. To ask only one Umbrella question, turn off All questions. Then turn Visible back on for Do you currently carry umbrella insurance?
Conditional questions
Some questions appear only when an earlier answer triggers them. In the builder, follow-up questions appear indented beneath the question that triggers them.
When a follow-up is triggered by a question in a different section, the builder links to that section. Use the link to review the triggering question.
Note: Indentation only shows how questions relate. It does not change which questions clients see or what triggers them.
Referred team member
The Contact section includes an optional question that lets the client pick a team member who referred them. Turn it on to see who sends you business.
Final Details
The Final Details page appears at the end of the form. Turn on the questions you want:
How did you hear about us?
Anything else you'd like us to know?
What is the best time to reach you?
Set up the form's settings
Open the Settings tab. Click Save [needs verification] at the bottom of each section after you make changes.
General
Name is for your team only. Clients never see it. The name also sets the form's public link, so each name must be unique.
Title is the heading clients see at the top of the form.
Lines of business sets which lines clients can choose.
Assigned team owner [needs verification] receives each submission. If you leave it blank, submissions go to your agency's team owner.
Choose whether clients see the assigned team member's name and email.
Warning: Renaming a form changes its public link. After you rename it, share the new link anywhere you posted the old one.
Branding
Upload your agency logo to show it on the form.
Notifications
Choose who gets an email for each new submission. You can add email addresses for people who do not use RiskAdvisor. Click Save notifications [needs verification] when you are done.
Tip: Check the recipient list after you change the assigned team owner. Make sure the right person gets the email.
Integrations
Send submissions to AgencyZoom as leads. You can assign leads to a user or an assignment group and choose a lead source. See Set up AgencyZoom in legacy QuoteForms for what Custom QuoteForms send.
Thank You page
Clients see the Thank You page after they submit. You can set:
The thank-you message.
What happens next, so clients know the next step.
The response time, so clients know when to expect you.
Share the form
Go to the Custom QuoteForms list.
Click Visit [needs verification] next to the form. It opens in a new tab.
Copy the link from your browser and send it to clients.
Custom QuoteForms work by link only for now. Embedding on your website is not available yet.
Tip: To build a similar form, click Duplicate [needs verification] in the form's menu. Then change only what is different.
Check your results
Open the form and fill it out yourself. Confirm only the questions you turned on appear. Confirm that required questions stop you until you answer them.
